Screwed by T-Mobile - How about you?

After getting our iPhones we cancelled our T-Mobile account. We called the customer service line cancelled our service and was told our service would end on a certain date.
Needless to say. T-Mobile did not cancel our account and continues to bill us. When we call the customer service line to inquire they claim that they have no record of our canceling the service. So we cancel again.
A quick Googling shows that this happens a lot to T-Mobile customers. With T-Mobile continuing to bill for service after a customer cancels and if the bills are refused sending the account to a collections agency.
Poor training, inept leadership, and bad processes can explain some of these incidents. But, with the number of stories I am hearing I wonder if something more is going on?
What has been your experience?
I should contact Robert P. Dotson and ask him if it is poor training, inept leadership, or bad processes that allows this to happen or if it is something more sinister.
